What is home health care?

Home healthcare is personalized health care in the comfort of your own home or place of residence. Our staff will coordinate with your physician to carry out all doctor orders and monitor your overall care while on services. Our team consists of nurses, therapists, social workers and home health aides. The goal of short-term home health care is to provide treatment for an acute illness or injury. It helps you get better, regain your independence, and become as self-sufficient as possible.

What are your business hours?

Our office hours are from 8:30 am to 5:00 pm, Monday through Friday. But we are readily available 24 hours a day - 7 days a week at (405) 778-6270 with both a RN and PT on-call at all times.


What should I have prepared for my clinician at Admission / Start of Care visit?

  • Insurance card/Medicare cards
  • All hospital DC paperwork that was sent home with patient
  • Have all medication bottles out and grouped together: This includes all new medications prescribed from surgery and anything patient was taking prior to surgery. Medications prior to surgery include all prescriptions, any vitamins/supplements, topical creams/ointments, eye drops, etc.
